Ø     Provide an OPERA brochure from you containing Surgicare of Wichita's web address.

Ø     Provide the patient their doctor's name, the date and estimated time of their surgery, and the name of the procedure available when they access OPERA on Surgicare's Web site.

Ø     The brochure will advise the patient that they should access the OPERA system at least 24 hours in advance of their surgery date.

Ø     Once the patient has successfully accessed the OPERA site from the Surgery Center's Main website page they will have "HELP" text available to assist them as they are completing the questionnaire.

Benefits for the Patient:

They can complete the Clinical History Questionnaire online -- when and where it's convenient for them.

Patient Users:

Patients will access the Online Clinical History questionnaire from Surgicare's web site and will be able to do the following:

-    Submit an Online Clinical History Form

-    Access Online Help text information

-    Print the Online Clinical History Form

     (prior to submission of the form)

-         Access and print the Surgicare's Pre-Op Instructions

-         Access a map to locate Surgicare of Wichita

Patient's without access to a computer or the Internet should be provided an OPERA paper form to complete and FAX or bring to Surgicare.
